This service is only available to patients registered with Chawton House Surgery. You do not need any login details to use this service.

When you submit an eConsult, this will be triaged by one of the GPs within 2 working days to see what next steps need to be taken.

This means you may be contacted by the GP or by the Patient Services Team who may call to book you in for a non-urgent appointment.

If you feel your query is urgent and cannot wait 2 working days, please call the surgery on 01590 672953.

When submitting your request, it is important to submit as much information as possible so that we can prioritise your request accordingly. If you have 2 unrelated problems, please submit 2 requests.

If you are submitting a form on behalf of someone else, including children, please make sure that when completing the personal information form that you tick the box saying ‘I’m filling the form on behalf of another person OR I am a care home worker’.
