Attention Deficit Hyperactivity Disorder (ADHD) and Autism Spectrum Disorder (ASD) Assessments
If you would like to be referred for an Attention Deficit Hyperactivity Disorder (ADHD) or for an Autism Spectrum Disorder (ASD) assessment, please read this page and complete the necessary steps. All referrals need to be made by the Surgery, however you do not need an appointment with a GP to do this.
- Pick a provider – Referrals for assessments can be made through local or national providers as a ‘Right to Choose’ referral.
Right to Choose – In England, under the NHS, you have a right to choose your mental healthcare provider and your choice of mental healthcare team. This means that, for instance, should you decide the waiting time for your assessment is too long, then you can choose alternative providers. Pick a provider that you wish to be referred to – make sure that they are authorised to prescribe medication. Input this choice on the form below when asked.
NOTE: CAMHS (Child and Adolescent Mental Health Services) have advised avoiding ADHD 360.
- Submit a pre-referral form to the Surgery – Fill in either the ADHD or ASD pre-referral forms below (or both if you wish to be assessed for both) and press submit.
ADHD form – 18+ ADHD form – under 18
ASD form – 16+ ASD – Under 16s Form
This will then be turned into a referral by the surgery and sent off. Your chosen provider will be in contact with you directly.
