GP Letters of Support

At Chawton House Surgery, we are committed to supporting our patients and understand that medical information may be needed when applying for benefits, housing, travel passes, workplace adjustments or other services.

We appreciate that these applications can be stressful, and we want to make the process as straightforward as possible.

To ensure requests are handled appropriately, we ask that any organisation requiring medical information contacts the practice directly with a formal request.

Unfortunately, we are unable to provide letters of support directly to patients.

Please be aware that:

  • All requests must be accompanied by the patient’s appropriate consent.
  • We can provide factual medical information, but we cannot offer opinions or recommendations about eligibility for benefits, housing or other services.
  • Where we are unable to complete a request, we may offer a copy of relevant medical records instead, with your consent.
  • A fee may apply to certain requests for medical reports or information. Any applicable charges will be explained before the request is completed.

Housing Applications

We understand that housing difficulties can be stressful and may affect your health and wellbeing.

However, GP supporting letters are not normally required for housing applications, and we kindly ask patients to follow the council or housing association’s application process.

If you are applying for housing on medical grounds, you will usually be asked to complete a medical self-assessment form explaining how your current housing affects your health.

If the council or housing association requires further medical information, they can contact the practice directly with a formal request and your written consent.

We can then provide relevant factual information, such as:

  • Medical diagnoses
  • Severity of medical conditions, where documented
  • Prescribed medication or treatment

The council or housing association is responsible for assessing your housing needs and making decisions about your application.

What If I Have Been Asked to Obtain a GP Letter?

We appreciate that patients are sometimes advised to request a GP letter, which can cause confusion and frustration.

If this happens, we can provide a standard letter explaining our policy and how the council or housing association can request medical information directly from us.

Thank you for your understanding and cooperation.
